Roughly 10,000 searches a month ask whether creatine helps with weight loss. The meta-analyses give an answer that sounds like yes and reads like no: body fat percentage goes down a little, body weight goes up a little, and fat mass in kilograms mostly does not move. The fat-burning mechanism behind the claim is real in mice and has not shown up in the one human trial designed to find it. Every one of them reports a lower body fat percentage. Neither of the two in this table that report fat mass in kilograms found a statistically significant loss. The 2023 paper says it in its own conclusion: a very small reduction in body fat percentage "without a corresponding decrease in absolute fat mass."
A fourth pooled analysis, of 12 trials in adults under 50, did report a fat mass reduction of 0.73 kg alongside 1.14 kg of added lean mass (Desai et al., J Strength Cond Res 2024, PMID 39074168). The fat-mass evidence is therefore mixed rather than absent — but none of it describes weight coming off the scale.
Why the percentage can fall while fat stays put
Body fat percentage is fat mass divided by total body mass. Creatine reliably raises total body mass. So the percentage can drop with no fat lost at all:

Our arithmetic, applying the meta-analysed mass changes to an illustrative person. It is not a figure from any of the papers.
On those numbers, the denominator alone accounts for about three-quarters of the 0.28-point reduction across 143 trials. It accounts for much less of the 1.19-point reduction reported in adults under 50, so that result is not simply an artefact — but its authors still found no significant fat mass change behind it. The honest reading is that a percentage figure should not be quoted as fat loss unless the kilogram figure agrees.
The scale goes the other way
Across 143 trials the average body mass change was +0.86 kg. That is the opposite of what most people searching this question are hoping for, and it happens on the same days the store fills — we trace what that mass is made of, and why two measurement methods disagree about it, on does creatine make you gain weight.
Whether creatine helps preserve muscle during deliberate weight loss is a different question with even less evidence: two small randomised trials, pointing in opposite directions, which we set side by side on creatine while cutting.
The mechanism: strong in mice, absent in the one human test
The reason creatine is linked to fat burning at all is a line of mouse research. Thermogenic fat cells appear to use creatine in a futile cycle — phosphorylating and dephosphorylating it to burn energy as heat. In 2021, researchers at McGill and Harvard showed that deleting the enzyme creatine kinase B from mouse fat cells reduced thermogenic capacity and increased predisposition to obesity (Rahbani et al., Nature 2021;590:480–5, PMID 33597756).
That shows removing the machinery harms mice. It does not show adding creatine helps humans, and when a group at Maastricht tested exactly that, they found nothing. In a double-blind crossover trial, 14 young vegetarian adults — chosen because vegetarians have low creatine levels, where any effect should be largest — took 20 g of creatine monohydrate a day or placebo. Brown fat activation after cold exposure, measured by PET-MRI, did not differ: SUVmean −0.34 (95% CI −0.97 to 0.28, p = 0.254) (Connell et al., Nat Metab 2021;3:107–17, PMID 33462512; NCT04086381). Two of the mouse study's senior researchers are co-authors of the null human trial.
Two years later, a review in the same journal went further. Its authors, both at the Buck Institute, examined the experimental basis for the futile creatine cycle and concluded "there is currently no convincing evidence for a significant futile creatine cycle in these tissues" (Nicholls & Brand, Nat Metab 2023;5:21–8, PMID 36624158). The question is open in the basic science. It is not open in the direction of a human weight-loss effect, because nobody has measured one.
Who wrote the fat mass papers
The 2019 meta-analysis declares no conflicts of interest. The 2023 meta-analysis declares that its first author serves on the Scientific Advisory Board of Alzchem, a company that manufactures creatine, has conducted industry-sponsored creatine research and acts as an expert witness in creatine cases, and that a co-author is a scientific advisor to a creatine gummy company. Both statements are printed in the papers. The 2023 paper's headline finding is nonetheless the null one: no fat mass difference. What can honestly be said
- Body weight rises on creatine, by about 0.86 kg on average across 143 trials.
- Body fat percentage falls slightly, by 0.28 to 1.19 points depending on the population.
- Fat mass in kilograms did not fall significantly in the two meta-analyses that tested it in adults under 50 and adults 50 and over; a third pooled analysis did report −0.73 kg.
- Much of a small percentage drop can be arithmetic: adding non-fat mass lowers the percentage with no fat lost.
- The fat-burning mechanism is a mouse finding. The one human trial designed to detect it, at 20 g a day in vegetarians, found no effect, and a 2023 review found no convincing evidence the cycle is significant.
